Dr. Mundo Counters: Who Beats Dr. Mundo
Updated for patch 26.21. Ability details come from Riot’s official game data for this patch.
Dr. Mundo counters are top laners who cut his healing, deal damage that grows with his Health, or kite him so his huge regeneration never gets to matter. He’s a 125-range tank who heals a lot, ignores the first crowd control that hits him, and turns max Health into Attack Damage.
Our five were chosen by reading their patch 26.21 kits alongside his. Win rates weren’t used.

Why is Dr. Mundo hard to kill?
- Goes Where He Pleases (passive): resists the first Immobilizing effect, losing Health and dropping a canister he can pick up to heal; he also has much higher Health regeneration.
- Infected Bonesaw (Q): a thrown saw that damages the first enemy hit based on current Health and slows. Cooldown 4.
- Heart Zapper (W): damages nearby enemies over time and stores part of the damage he takes; the burst at the end heals him for some of it if it hits. Cooldown 17/16.5/16/15.5/15.
- Blunt Force Trauma (E): passively, bonus Attack Damage from max Health; active, a bag slam that hits harder the more Health he’s missing. Cooldown 9/8.25/7.5/6.75/6.
- Maximum Dosage (R): instantly heals part of his missing Health, then gives Move Speed and regenerates max Health over a long duration. Cooldown 120.
Mundo heals from almost everything. Regeneration is built into his passive, Heart Zapper gives back part of the damage he takes, the canister refills him, and Maximum Dosage is a two-stage heal with Move Speed on top. Then there’s the anti-stun. The first Immobilizing effect that hits him does nothing (he just loses some Health and drops a canister), so a single stun rarely stops him. What beats all that? Grievous Wounds on every hit, damage that scales with his Health pool, and champions who never let him reach melee range in the first place.
Who are the best Dr. Mundo counters in top lane?
Darius
Hemorrhage (passive) makes every attack and damaging ability bleed for 5 seconds, stacking to 5, and at max stacks Darius enrages with massive Attack Damage. Against a tank who wants a long fight, that’s ideal. The ultimate is a true-damage dunk that hits harder per bleed stack on Mundo and comes back right away if it finishes him. Apprehend (E) passively ignores part of the target’s Armor and actively hooks enemies in, and Decimate (Q) heals Darius off champions hit by the blade. Crippling Strike (W) slows as the target bleeds. Real talk, Darius doesn’t care how much Health Mundo has as long as the fight lasts five seconds.
Teemo
Teemo never lets Mundo use his Attack Damage. Blinding Dart (Q) damages and blinds, and a blinded Mundo swinging a bag at Teemo hits nothing, even with all the bonus Attack Damage from Blunt Force Trauma. Toxic Shot (E) poisons on every attack for damage on impact and each second for 4 seconds, chip damage that adds up against regeneration. Move Quick (W) increases his Move Speed until he’s struck, and the active sprint keeps going even after being hit. Mushrooms from his R carpet the lane, and each one Mundo walks onto slows and poisons him (the lane gets crowded fast). Guerrilla Warfare (passive) makes Teemo invisible after standing still. He has 500 range to Mundo’s 125, so every trade starts with Teemo hitting first and Mundo walking toward him through a minion wave.
Quinn
Quinn plays the same kite game from a different angle. Blinding Assault (Q) sends Valor to mark an enemy, hinder its vision and damage everything nearby, and Vault (E) dashes to an enemy, slows, interrupts briefly, then leaps back out near max range. She has 525 range. Valor keeps tagging targets, and Quinn’s first shot on a tagged Mundo hits extra hard, then hands her a burst of Attack Speed and Move Speed through W’s passive. Behind Enemy Lines (R) lets her fly around the map at great speed and ends with Skystrike. Is Infected Bonesaw a problem? Only if she walks in a straight line; it’s his one ranged tool.
Gwen
Gwen’s passive is the sort of damage a Health stacker hates. A Thousand Cuts adds magic damage based on the target’s Health to each attack and heals Gwen off champions, so the bigger Mundo gets, the more each of her hits takes. Snip Snip! (Q) cuts up to 6 times in a cone, true damage in the center. Hallowed Mist (W) means she can only be targeted by enemies inside the mist, which blocks Infected Bonesaw from outside it. Her E is a short hop that leaves her attacking faster, from farther, with extra magic damage per hit. Needlework (R) slows and stacks the passive across three casts. Honestly, a Mundo who builds Health is building her damage for her.
Fiora
Fiora wins long fights because she heals too. Her passive keeps revealing weak points on Mundo, and each one she strikes gives her Health and Move Speed, so she trades evenly against his regeneration instead of losing the war of attrition. Bladework (E) speeds up two attacks, slowing with the first and critting with the second. Her Q is a quick stab-dash. Riposte (W) parries damage and crowd control for a moment, and its counter-stab stuns if it parried something immobilizing. Grand Challenge (R) exposes four points at once and heals her and her allies once all four are struck. To be fair, she still needs Grievous Wounds to keep up later.
How should Dr. Mundo counters play the lane?
- Dodge Infected Bonesaw. It hits the first enemy for damage based on current Health and slows; stay behind minions.
- Expect the first stun to fail. Goes Where He Pleases resists the first Immobilizing effect, so lead with a slow or a cheap stun.
- Step out of Heart Zapper’s burst. He only heals from the stored damage if the burst hits an enemy.
- Pick up or deny the canister. Walking over it heals him and lowers his passive’s cooldown.
- Hold burst for after Maximum Dosage ends. It’s a 120 second cooldown, and his regeneration drops back to normal afterward.
Which items help Dr. Mundo counters?
Grievous Wounds, early and always. Executioner’s Calling applies 40% Wounds on physical damage for 3 seconds, and Chempunk Chainsword or Mortal Reminder keep it going later; mages get it from Oblivion Orb and Morellonomicon. Damage that scales with his Health helps too: Blade of The Ruined King takes a slice of current Health on every hit, and Lord Dominik’s Regards adds bonus damage against high bonus Health. Every one of these is in our LoL item reference.
When is Dr. Mundo weakest?
Early, before his Health pool grows. Blunt Force Trauma turns max Health into Attack Damage, so a level 3 Mundo with starting items hits far softer than the late game version. Want to snowball him? Do it before his first Health item, while Blunt Force Trauma still has very little max Health to convert and his bag slam barely tickles.
And while Maximum Dosage is down. Without it, his healing is mostly passive regeneration plus Heart Zapper, which Grievous Wounds cuts hard (for what it’s worth, that’s the moment to commit your burst).
New to top lane? Our beginner’s guide to roles covers it. More champion breakdowns sit in the guides page, and his skins are on the Dr. Mundo skins page. Put simply, Dr. Mundo counters cut the healing and punish the Health.
